Clinic Policies

Please review the following information before your visit with us. 

Appointment cancellations

Your scheduled appointment time has been reserved for you. Every effort will be made to confirm the appointment with you. We require 48 hours (2 business days) notice to cancel or reschedule your appointment. Late cancellations without valid reason or no-shows may be subject to a $150 fee.

Communication

Our office uses email to communicate administrative issues only. All medical concerns must be addressed at a medical appointment (conducted in-person or virtually). 

Non-insured
Services (not
covered by OHIP)

OHIP does not pay for all services that patients request from physicians. Services that OHIP does not pay for are called “uninsured services” and physicians are prohibited from billing OHIP for these services. All uninsured services must be paid in full when rendered; office staff will provide a receipt for services provided. Our clinic charges the following for these uninsured services:

1. School forms/camp forms/sick notes/plane notes: $35

2. Lost prescriptions: $30

Co-parenting with appointments

When possible we encourage both parents to attend the appointments in cases of separation. If this is not possible, our clinic will communicate with the parent with whom the child was brought to the office. It is the responsibility of the caregiver who attends the appointment to inform the other caregiver of investigations, diagnoses, and treatment plans. We would respectfully request that our clinic staff not be involved in any interpersonal disputes.
